Council Forms

Council forms are posted in PDF format.  To obtain the free Adobe reader (used to read PDF forms) you can go to Adobe's website. If you have questions about the forms, contact Autumn at 1-800-634-4473.

Travel Reimbursement
Travel reimbursement forms are passed out at each Council meeting.  If you need a form for other Council-related travel or didn't receive a form at a Council meeting, you can use one of the forms below to request reimbursement for your Council-related travel.  

• Travel Reimbursement Form (Print-Only) - to use this form you must have Adobe Acrobat Reader (any version) installed on your computer.  You can print your completed form, but you can't save it.
• Travel Reimbursement Form (Print & Save) - to properly use this form you must have Adobe Acrobat Reader 8.1

Conflict of Interest
Conflict of Interest forms are completed at each January meeting by all Council members.  If you missed the January meeting, please complete the Conflict of Interest form after reading the Conflict of Interest policy.

• Conflict of Interest Policy - Read policy in its entirety before completing the form.
• Conflict of Interest Form - Print form, complete and sign.  Send the completed form to: Linda Walling, DDC Membership Coordinator, PO Box 48314, Olympia, WA 98504-8314.

Council Member Conference or Training Request
Council member conference or training request forms need to be completed when a council member is requesting the Council pay for a conference or training related to the Council member's participation on the Council.  Requests must be signed by the Council member making the request
